Downgrade your legacy GTA 5 to 3586, then reinstall 5Real and it will fix it.

5real was ended by the author themselves in May 2025 because it had never undergone long-duration, large-scale, high-speed stress testing. This made it impossible to detect memory overload issues. Too many props that couldn't be quickly reclaimed by memory were added, and even with the "realism script" installed, the problem persisted. The testing methods were as follows:
1. Driving a high-speed vehicle around the map once to load resources into memory.
2. Driving a high-speed vehicle and crashing into other vehicles.
3. Making high-speed turns in downtown areas with many tall buildings.

I have installed the september release on a fresh GTA 5 installation with only NVE, but I experienced several issues, including texture/model pop-in, instability, crashes, blips disappearing from the map, and the realism script not functioning correctly.
Although I have followed the tutorial, I believe I will need to wait for a new version.

BRO SOME OF THE OIV FILES DOESENT WORK AND SOME DO WHEN I TRY TO EXTRACT IT IT SAYS DOME ABOUT CANT BE IN SOURCE AND DESTINATION
Mod is broken, don't waste time, this has been talked about just search for 5real in here and look at the threads
You shouldn't be using a computer if you don't know how to extract some files in parts, plus, the mod is broken it will crash your game also don't bother
